If we are constantly losing hair, how does it get long?

We lose full strands of hair at the root and partial strands through stress and natural attrition. Chemicals, illness, poor nutrition, over processing, etc all can cause hair loss . Rarely do we lose all of our hair and if so the loss is at the follicle. New hair is constantly being regenerated and often at a faster rate than natural hair loss. Your scalp contains hair of many lengths you just don't notice it. Hair grows from the follicle out. It does represent cell death in actuality and the longer the hair the longer the "chain of dead cells". Hair growth can continue long after death as can nail growth as the dead cells are pushed from the follicle.
